Life Insurance is available for eligible employees only. It is not provided for retirees, dependents, persons who are making COBRA self-payments, or utility / REA / Electric Cooperative Members.
The Life Insurance benefit amount is $20,000.

Accidental Death and Dismemberment (AD&D)
AD&D Insurance is available for eligible employees only. It is not provided for retirees or dependents or utility / REA / Electric Cooperative Members.
The full AD&D benefit of up to $20,000 is paid for loss of life. This amount is in addition to the Life Insurance Benefit shown above. Up to $20,000 will be paid for dismemberment, as specified in the SPD.

Benefit Payment and Beneficiaries
The death benefit will be paid in a lump sum to your beneficiary. You can designate your beneficiary or beneficiaries by filling out the Death Beneficiary Card (see the Family Enrollment Card / FEC) and sending it to the Fund Office.
If you haven't named a beneficiary or if your named beneficiary dies before you, your Life Insurance will be paid to the first of the following successive classes of survivors: your spouse, your children, your parents, your brothers and sisters, or your estate. If there is more than one survivor in the class payment is made to, the survivors in that class will share equally.
