Extension of Benefits
(This extension of benefits provision doesn't apply to any person who is
covered under Medicare or under any other welfare fund, group plan or plan
sponsored by any employer at the time his eligibility under this Plan terminates.)
- If, on the date his eligibility terminates, a covered person is totally
and continuously disabled as the result of substance abuse or a mental or
nervous disorder, eligibility for Mental Health and Substance Abuse Benefits
will be continued beyond that person's termination date.
- Benefits will be payable only for treatment of the condition that caused
the person's total disability and only to the extent that they would have
been payable had his eligibility not terminated.
- Benefits will be continued for the person for up to 3 months beyond termination
of his eligibility. This extension will cease on the first to occur of the
following dates: (1) the date the person is no longer totally disabled;
(2) the date the person becomes covered under Medicare or under any other
welfare fund or any group plan or any plan sponsored by an employer; or
(3) the end of the 3-month period following the date his coverage terminated.
No extension of benefits applies to the Lineco Member Assistance Program
(MAP).
