The Mail Service Prescription Drug Program is also administered by Express Scripts. The Express Scripts mail-service pharmacy will provide long-term or maintenance prescription drugs to Plan participants at negotiated discounted prices. You can use this Program to buy the prescription drugs needed on an ongoing basis for conditions such as high blood pressure, high cholesterol, arthritis, diabetes, etc. It is not for drugs taken on a short-term basis, such as antibiotics.

Covered Drugs - Covered drugs and medications under the Mail Service Program are the same as those covered under the Comprehensive Benefit and the Drug Card Program.

Payment Provisions - The Comprehensive Benefit's regular deductible and payment percentage provisions don't apply to prescriptions filled by the mail-service pharmacy. Instead, you pay the following co-pays to Express Scripts each time you order up to a 90-day supply of your medication:

If you choose a brand name drug instead of an available generic, you must pay the difference in cost between the brand name drug and its generic equivalent, in addition to the $10 generic co-payment.

The Plan pays 100% of the rest of the prescription cost to Express Scripts.

Your co-pays do not apply toward satisfaction of any deductibles under the Comprehensive Benefit or toward meeting any out-of-pocket maximums. Benefits paid by the Plan to Express Scripts for covered drugs do not apply to the Comprehensive Benefit lifetime maximum benefit.

About Preferred Drugs - "Preferred" prescription drugs are brand name medications that have been evaluated by Express Scripts physicians and pharmacists and determined to be the most effective for treatment of certain conditions for most patients, and that are reasonably priced. They are often referred to as "formulary" drugs. The list of preferred drugs is frequently reviewed and updated as new drugs are introduced. If more effective or safer drugs that are reasonably priced appear on the market, they may be added to the list or replace a drug already on the list. For the most recent information about the formulary or the status of a specific drug, contact Express Scripts.

We recommend that you bring your copy of the formulary when you visit your doctor so that together you can make decisions about covered medications and alternative medication before the doctor writes the prescription. If your doctor prescribes a brand name drug not listed in the formulary, consider asking whether a formulary drug would be just as effective. But remember that the list of preferred drugs is not a list of covered drugs, and the fact that a category of drugs or a specific drug appears on the list does not mean that it is covered by Lineco. For example, Lineco excludes drugs for birth control, infertility and obesity even though they may be on the formulary.

About Generics - The generic name of a drug is its chemical name. The brand name is the trade name under which the drug is advertised and sold. Both generic and brand name drugs must meet the same Federal requirements for safety, purity and strength. The main differences are the name of the manufacturer and the price—generic drugs are usually much lower in cost. To save money, ask your doctor to approve generic substitution when possible on the prescription form.

Ordering Prescriptions - Your Express Scripts packet includes a brochure explaining the program, order forms, and an envelope addressed to Express Scripts. Your co-pays can be paid by check or money order or you can charge them to a major credit card. Your medications will be delivered to your home by first class mail or UPS within 14 days. Express Scripts pays for the postage or shipping cost. You can order authorized refills by mail, telephone or on the internet (www.express-scripts.com). Express Scripts will fill the order and your co-payment will be charged to your credit card. You can transfer your prescription from a retail pharmacy to the mail-service pharmacy by logging onto Express Scripts' internet site and asking them to obtain the script directly from the physician. Or, you can call Express Scripts at 1-(877)-327-0568 and ask them to call the physician so that the prescription can be transferred.

Your first mail-service order takes longer to process because Express Scripts creates a profile for each mail-service customer. This is one-time delay ONLY. You will receive your subsequent refills and new prescriptions more quickly.
