Dependent Child Turns 19
Your child's 19th birthday may not seem like a huge milestone, but there is one major change affecting your benefits under the Plan. Dependents who are age 19 through age 24 must provide proof of full-time student status on a form supplied by the Fund Office to retain coverage as your dependent. This evidence must be supplied once per school term (semester or quarter).

If your dependent child is a full time student, you must submit proof of full-time student status to the Plan Office each semester for continued coverage through age 24. When your child ceases to be a full time student, you must notify the Plan Office as soon as possible. You will become liable for any health claims paid by the Plan for any services provided after your dependent ceases to be eligible.

If you do not supply evidence of student status or if your dependent child is no longer a student, the child may be eligible for continued coverage under COBRA for up to 36 months if the Fund office is notified within 60 days.
|
|
If your child becomes totally disabled prior to turning age 19, the child may be eligible to continue as a dependent under your coverage. Contact the Fund office for assistance.
